If I could only run two tests on every person I meet, these would be it. Not because they're the most expensive — because they're the most important.
Unlike large functional testing programs where results are reviewed by an unknown provider on a team, you know exactly who is interpreting your labs. That means your results are read in the context of your health, your goals, and your story — not processed by a stranger. In 15 years of functional medicine practice, the two tests in this panel are the ones I run on almost every patient.
Standard lipid panels are designed to screen for disease — functional panels are designed to optimize health. There's a meaningful difference. This screen measures the markers that give a fuller picture of cardiovascular risk, interpreted using functional ranges rather than standard reference ranges alone.
A standard cholesterol panel measures total cholesterol, LDL, and HDL — useful starting points, but an incomplete picture of cardiovascular risk. This panel goes significantly deeper — measuring LDL particle size and number (small dense LDL particles carry more risk than large buoyant ones), ApoB (one of the most accurate single predictors of cardiovascular risk available), and Lipoprotein(a) — a genetically determined risk factor affecting 1 in 5 people that is not captured on a standard lipid panel. Lp(a) is not modifiable through lifestyle alone, but knowing you have it meaningfully shapes how you approach other risk factors. Also includes non-HDL cholesterol, VLDL, and remnant lipoproteins.
Homocysteine is one of the most important and most overlooked markers in cardiovascular medicine. Elevated homocysteine is independently associated with heart disease and stroke, cognitive decline and dementia, blood clot formation, and damage to the lining of your blood vessels. It's also a direct window into methylation — your body's ability to repair DNA, regulate neurotransmitters, detoxify, and process hormones. The good news: elevated homocysteine is one of the most treatable findings in functional medicine.
Especially if you have a family history of heart disease, have never had advanced lipid testing, are concerned about brain health and cognitive aging, or simply want these cardiovascular markers interpreted by a functional medicine physician who will tell you what to do with the results.
This is also an ideal first step for anyone who has had a standard cholesterol panel and been told their numbers look fine — but wants to know what the full picture looks like.
